RECONSTRUCTION OF CHRONIC BONY EXPOSURE IN LOWER EXTREMITY BY MUSCLE FLAP COVERAGE

Abstract
|
|
|
To reconstruct the chronic bony exposure with extensive soft tissue defects of the lower extremity, the muscle flap coverage is preferred to cutaneous flap transfer.
7 lower extrmities in 6 patients of postburn wide bony exposure of lower extremity accompanying osteomyelitis were successfully reconstructed by various types of muscle flaps and proper administration of sensitive antibiotics. 1 patient developed recurrent infection at the postoperative 3 months, and the overall 83% of controlled infection rate were showen in this series. The average follow up period was 1.7 years in these 6 patients, all patients are currently ambulatory. This results demonstrated the effectiveness of muscle flap coverage in chronic bony coverage of lower leg with large area of soft tissue defects.
As a newly designed retrograde flow muscle flap. Reverse Extensor Digitorum Longus muscle flap trabsfer up 3 cases showed good results and it could be expected good method in moderate to large sezed defects of distal third & middle third of tibia, ankle. nkle.
